TERMS AND CONDITIONS

By participating in the Born 2 Hoop Holiday Camp you agree to the following waiver (based in part on Basketball Victoria’s).

Note: the ‘I’ in the waiver below refers to the player, parent or legal guardian – as appropriate.

I consent to receiving emergency medical treatment, which may be deemed advisable in the event that suffer injury, accident and/or illness.

I understand that I am required to be in attendance throughout camp.

I recognise that the sport of basketball is physically demanding and carries some risk of injury. I declare that I am physically fit and able to compete in the sport of basketball and have not been told otherwise by a medically qualified person.

Both personally and for my legal personal representatives I release and forever discharge Born 2 Hoop Basketball Program, from all liability, claim, damages, costs or expenses including all injuries that my child may suffer before, during or after a basketball skills session. I understand that this waiver includes all claims based on negligence or the action or inaction of any of the above-mentioned parties.

I agree to images of myself/my child engaged in the Born 2 Hoop Holiday Camp to be used for legitimate promotional purposes in relation to the Born 2 Hoop Basketball Program eg. Website, promotional flyers, and newsletter.
